Responsibilities:
– Evaluating the requirements of individual service users and their caregivers for social care services.
– Conducting crisis and risk evaluations.
– Crafting a personalized support plan based on assessed needs and budgetary considerations.
– Promoting the utilization of direct payments for individuals to manage their care independently, while offering or guiding them towards necessary support.
– Identifying and addressing the needs of vulnerable individuals at risk of significant harm, while respecting the autonomy of those with decision-making capacity.
– Cultivating and enhancing cooperative partnerships with various agencies, both public and independent, to uphold the principles of collaborative work as outlined in “Working Together.”
What we are looking for in a Social Worker:
* Social Work England registration
* Eligible to work in the UK
* Hold a full UK licence
* Post-qualified experience
* Degree level or equivalent in Social Work
