We are looking for a motivated and well organised individual to Co-ordinate the anticoagulation team. This individual needs to work autonomously and play a pivotal role within the anticoagulation team
Co-ordinate the day-day management of patients newly commenced on anticoagulation in the out-patient anticoagulation clinic by:
Ensuring attendances to clinic are monitored and that patients who DNA are followed up and that GP and nursing team are advised accordingly of non-attendance or suspected non-compliance.

Print off clinics lists for nursing staff
Telephone patients to advised of INR result and dose of warfarin that needs to be taken (as instructed by nursing team) ensuring that this information is given at a level that is understood by the individual patient.
