Exciting Opportunity: Young Person’s Resilience Worker –
Supported accommodation Lead.
Are you skilled in engaging young people through a variety of interventions, utilising a young person-centered approach to empower them in reducing risk, enhancing resilience, and leading safe, happy, and healthy lives? In this role, you'll be an integral part of our multi-disciplinary team at Positive Futures Young Person service, delivering high-quality provision.

Start Date: We're looking to welcome the right person to our team ASAP.
Hours: Full Time 37.5 hours per week
Full Time Salary: £27,449.52 - £31,529.41 Pro rata (dependent on experience)
Contract Type: Fixed term until 3st of March 2025
Responsibilities
About the Role:
To excel in this role, you will need to:
Supporting Service Users:
1. Assist service users from the moment they enter the service through their treatment and recovery journey.
Providing Comprehensive Care:
2. Conduct screening, assessments, and psychosocial interventions.
3. Develop integrated care plans and offer post-treatment support, including transfer of care.
Safeguarding:
4. Identify and respond effectively to potential safeguarding issues.
Reducing Harm:
5. Focus on reducing drug and alcohol-related harm, especially among young people in supported accommodation.
Building Therapeutic Relationships:
6. Develop therapeutic relationships and deliver a personalized package of care, including evidence-based psycho-social interventions.
7. Engage in 1-to-1 key working and group work across supported accommodation sites in Gateshead.
8. Implement harm minimization and brief interventions.
9. Contribute to whole-family interventions.
Assessment and Planning:
10. Conduct comprehensive service user assessments.
11. Develop individual care plans that outline clear goals for addressing substance use and related outcomes.
Collaboration and Safeguarding Leadership:
12. Work closely with your service’s Safeguarding Lead to ensure swift and effective resolution of issues and concerns.
Building Partnerships:
13. Establish relationships with emotional well-being and mental health providers to enhance pathways in and out of service.
14. Contribute to improving our toolkit of interventions related to emotional well-being and mental health needs.
Advocacy and Multiagency Collaboration:
15. Provide advocacy for young people by actively participating in relevant multiagency meetings, including Child Protection meetings, Complex Case Review panels, and Team Around the Child meetings.
About You:
16. Experience engaging with young people individually and in groups
17. Understanding of modern youth issues and safeguarding concerns
18. Knowledge of drug/alcohol issues and strong partner relationship skills
19. Excellent time management, initiative, and collaboration abilities
