As an Advance Choice Document facilitator, you will provide highly specialist interventions to work with service users who have previously been detained under the Mental Health Act to co-produce Advance Choice Documents (ACDs) with all the services they use across the Trust. The post holder will lead the development of co-produced, good, evidence-based practice for production of ACDs in mental health services and assist in delivering implementation strategies for their completion, access and use, including awareness raising and staff training. The post holder will work across inpatient, crisis and community mental health teams to lead directly on the development of advance choice documents and to facilitate this process through awareness raising and training for all stakeholder groups.
