The post holder carries responsibility for the assessment of coordinating care needs and theirdevelopment, implementation and evaluation of programmes of care for Adults with mental healthproblems that are referred to the First Response and Recovery and Wellbeing Team. The post holderis responsible on a day-to-day basis to the Clinical Lead but ultimately accountable to the TeamManager for the overall service. In addition, they will be a member of a multi-disciplinary team andwork in close co-operation with GPs, wards and Primary Health Care based professionals. They willalso be responsible for the maintenance of standards of care within the community mental healthservice. They will also be responsible for ensuring that an integrated service is provided to theirclients and ensure that a good inter-disciplinary working environment is maintained. KEYRESPONSIBILITIES 1. To hold patient caseloads and maintain effective individual care regardingmental, health and social care needs by: - Holistic assessment of client problem/needs. - Planning,implementation and evaluation of interventions. - Clear, accurate and concise documentation ofclient care 2. To participate in the initial assessment of clients to determine appropriateness for theCommunity Mental Health Service and to liaise with referring agencies to identify alternativeagencies as appropriate to meet the clients needs. 3. To participate in Team and Clinical Meetings,initiating and discussing new methods/models of care and in the introduction of research or trialswithin the area. 4. To be fully aware of the importance of effective risk management, theundertaking of clinical risk assessment and the on-going monitoring of the clients mental state toevaluate risk either to themselves or towards others.
