We are seeking a Clinical Pharmacist to provide support to two community rehabilitation wards at Petersfield Community Hospital, and to regularly work on the Older Persons Mental Health wards at Gosport War Memorial Hospital and Elmleigh Adult Mental Health Unit in Havant.
Your work will focus on enhancing patient safety in medicines practice including medicines reconciliation, providing clinical professional advice to prescribers as part of a multi-disciplinary team, discharge planning, clinical audit, and teaching. As such you will need to be an excellent and empathetic communicator, with a thorough attention to detail and the ability to function in a busy environment.
Applications are welcome from pharmacists in any branch of the profession and at any stage of their career as full training will be tailored to the specific needs of the post holder. No previous experience is required of working within mental health, or on physical health wards, as you will be supported and mentored by experienced pharmacy colleagues.
The in-house Education & Training pharmacy team co-ordinate opportunities available for comprehensive and fully funded training. This includes the clinical pharmacy diploma, independent pharmacist prescribing, psychiatric certificate and diploma College of Mental Health Pharmacy courses Psych 1 and 2.
Main duties of the job
The post holder will be expected to work with the Principal Pharmacist to develop clinical pharmacy services within the division. They will be responsible for implementing and monitoring the Trust's clinical pharmacy standards as well as ensuring the clinical service is of high quality and patient focused. They will help to identify gaps and problem areas in the service and use appropriate methods including quality improvement to improve the service.
In conjunction with the clinical pharmacy leads and others in the team, the post holder will deliver an efficient, high quality, professional and well-coordinated clinical pharmacy service to physical health and mental health inpatient wards that meets all statutory, regulatory and NHS requirements. This will include the provision of prescribing advice to medical teams and medicines management to the multidisciplinary teams and patients at ward/clinic level.
Key areas of practice will be to:
1. Utilise pharmaceutical expertise, working in partnership with patients, carers and healthcare professionals to minimise medicines-related harm and facilitating medicines optimisation.
2. Provide clinical pharmacy advice and participate in the pharmaceutical services to the Portsmouth and South East Hampshire Division.
3. Monitor drug usage and promote cost effective prescribing within the Division.

Person Specification
Qualifications
* Masters Degree in Pharmacy plus pre-registration/ foundation year training.
* Registered with the General Pharmaceutical Council.
* Postgraduate Diploma (or MSc) in Clinical Pharmacy Practice or equivalent experience (or willing to work towards this).
* Postgraduate Certificate in Psychiatric Therapeutics (or willing to work towards this)
Experience
* Post registration experience working as a pharmacist.
* Demonstrate the ability to recommend, substantiate and communicate medicine related information appropriately to mental health patients, carers, and clinical staff, as well as those in physical inpatient wards.
* Experience of delivering training and education.
* Experience of working in a constantly changing environment.
* Post-registration experience working in mental health and/or community health services.
* Experience of ward based clinical pharmacy.
* Experience of working with older persons.
* Monitoring and advising on drug expenditure.
* To have undertaken clinical audit.
