As part of the Berkshire West Community Respiratory Service, you will provide high quality, evidence-based care for patients undertaking Cardiac or Pulmonary Rehabilitation. The role includes assessing, planning, implementing, monitoring, and evaluating exercise programmes according to the needs of the referred individuals. You will be based in the community (at leisure centres/community centres or at the client’s home) as a lone worker and with other members of the multidisciplinary team.
As a team player, you will be working within the Cardiac and Respiratory Specialist Services (CARSS) including the Respiratory Services (and Pulmonary Rehabilitation) Heart Function Service and Cardiac Rehabilitation, all based at Coley Clinic. Main duties of the job
1. Work as part of an integrated team providing community-based Cardiac and Pulmonary Rehabilitation.
2. Work as part of the Cardiac and Pulmonary Rehabilitation team to deliver the rehab programmes including assessment of patients and supervision of group exercise classes.
3. Provide Home Exercise Programmes for those who are unable to attend the community-based programme.
