An exciting opportunity has arisen for a full-time Band 6 Primary Care Mental Health Practitioner to join our East Neighbourhood Mental Health Team for 6 months maternity cover on a fixed-term contract.
Within BSMHFT, our community mental health services are well-established and as part of the BSOL community transformation programme, this is an exciting opportunity to be at the heart of national change and developments in the delivery of community mental health care.
We are looking for enthusiastic and dynamic individuals who are able to work within a multi-disciplinary team, alongside GPs and other primary care professionals, striving to achieve the best outcomes for individuals within the local community.
You will be responsible for:
1. Providing mental health triage, assessment, and outcomes-based intervention to meet service users' needs within the Primary Care Networks across East Birmingham.
2. Identifying need and the best course of advice, support, or intervention.
3. Promoting psychosocial wellbeing by identifying and supporting pathways for service users and carers.
4. Supporting and advising colleagues within the PCN and across local neighbourhood multi-agency teams.
5. Ensuring robust communication across mental health providers, VCSE organisations, and within primary care.
