Qualified Social Worker - Hospital Discharge
Based at Princess of Wales Hospital
Pay rate £35 per hour payrolled via Umbrella (PAYE equivalent available)
A Qualified Social Worker with a current DBS is required to support the hospital social work function at the Princess of Wales Hospital, Social Services department. You will be required to assess people on hospital wards in a fast-paced environment which can be challenging and stressful; however, it is rewarding. This role is offering an ASAP start and a 3-month contract. Pay rate is competitive and subject to experience. On-site working is required for this role. Experience PQ is required for this role in facilitating discharges, as well as candidates who have completed their CPEL.
You will need to be able to complete assessments in line with the Social Services and Well-being Act. You will need to be confident in applying the principles of the Mental Capacity Act and able to identify triggers for CHC. Knowledge of using the WICCis system would be helpful in this role. The team works closely with the multi-disciplinary team within the hospital, generally working with adults over 18 years with physical disabilities and older people, and at times co-working with people who have existing care managers in Learning Disabilities or Mental Health services. The team works mostly with inpatients but also collaborates with the MDT in A&E and SAU to prevent unnecessary admissions to hospital. The successful candidate will work closely with MDT colleagues and will be able to use an outcome-focused, person-centered approach to complex case work. You will be part of a team that works in challenging situations and supports each other to obtain positive outcomes for all involved.

* A minimum of 6 months experience of working in the UK as a registered social worker
* A relevant Social Work Qualification
* Registration with Social Care Wales / Social Work England
* Enhanced DBS, registered on the update service or be happy to apply for a DBS
* 3 Years of referencing
* Driving license with access to a car
