An exciting job role has recently become available at the West London Mental Health NHS Trust. They are looking for a new member for their Forensic Team.
The role’s purpose is to assess, plan and review health and social care needs under the Care Programme Approach (CPA) for a group of service users with an offending history. The post holder will work as a Care Co-ordinator as part of a multi-disciplinary team This will include participating in an evening out of hours rota to provide support and guidance to service users, carers and other services.
Requirements:
* To manage a caseload of clients, in close collaboration with other members of the Multidisciplinary team, within the CPA framework and to agreed local and national standards.
* To ensure that continuity of care is maintained and that appropriate and timely plans are made for the discharge of clients from the inpatient faculties ensuring Section 117 and CPA responsibilities are discharged.
* To provide joint assessments and ongoing joint work with generic mental health teams where there is a forensic component to the service user’s presentation.
What we are looking for in a Social Worker:
* Social Work England registration
* Eligible to work in the UK
* Hold a full UK licence
* Post-qualified experience
* Degree level or equivalent in Social Work
