This is an exciting opportunity to join Hawthorn Recovery Unit as a Nursing Care Coordinator, managing a complex caseload and working closely with colleagues in the multidisciplinary team to provide high quality care.
Working at Hawthorn Unit you will:
Carry out assessments independently including risk assessments and capacity assessments.
Develop care plans using Dialog Plus working in co-production with service users and family
Provide treatment interventions on a group and individual basis
Work with your nursing colleagues to ensure efficient and safe medication management
Provide supervision and training for staff and students
Carry out physical health checks including vital signs and ECG's
Support discharge planning and help service users and family members to develop support and engagement that will continue after discharge.

In order to meet the needs of the Trust you may be required from time to time to work at different locations to your normal place of work. This may mean that you are required to work at any location that fall under Barnet, Camden, Enfield, Haringey or Islington. The Trust reserves the right to require staff to work at such other places or locations as it considers reasonable and necessary on a temporary or permanent basis.
Hawthorn Recovery Unit is a small busy team where working together positively is a priority.
You will be responsible for your own allocated caseload but also work to support across the whole team caseload and to maintain the safe and therapeutic environment at the Unit.
There are lots of opportunities to develop enhanced skills through joint working and training provided through Team Workshops.
We carry out physical health screening for all new referrals and phlebotomy skills are desirable.
You will also have the opportunity to take part or lead Quality Improvement initiatives
You will support less experienced staff and students through supervision and informal training
