The capacity team are responsible for Patient Flow, placing the right patients in the right place at the right time and supporting in actioning targets relating to the ED 4 hour wait to uphold patient quality.
The post holder will be responsible for providing efficient, effective and comprehensive clerical support to the Capacity Team to assist in facilitating patient flow.
Responsibilities include:
1. Communicating effectively with all departments regarding updates on specific patients, supporting patient flow.
2. Updating and maintaining relevant spreadsheets, recording accurate information pertaining to referrals and breaches.
3. Recording accurate information in relation to actual and potential bed availability and communicating this information to all key contacts internally and externally when appropriate.
4. Updating information systems for use by internal and external teams as appropriate.
5. Maintaining and updating MDT databases to ensure the timely and accurate completion of data on multiple systems relating to bed utilization and other required data reports for daily and weekly information and managerial reports.
6. Escalating any delays/concerns to relevant managers/escalation leads.
7. Booking ambulances in keeping with DOH guidelines and tracking and monitoring ambulances for discharges.
8. Liaising with alternative patient transport agencies as appropriate.
9. Supporting in the implementation of the trust escalation policy.
10. Communicating in a manner that placates potential complaints and concerns.
11. Liaising and communicating with CCG and Social Care Partners as appropriate and necessary.
12. Providing information to support in the completion of the weekly operational.
