Futures+ is a unique and important team within East Riding Adult Specialist Services. We work in accordance with the Care Act 2014. We provide assessments, care & support planning and reviews for adults with learning disability and/or autism. We work with people with complex needs. Our service users require a high level of support in many aspects of their daily life. They also rely on a wide range of health and social care services.
Futures+ is based at County Hall, Beverley. There is opportunity for hybrid working. We are a very friendly and extremely supportive team with diverse experiences, knowledge, and skills. We are looking for a confident, competent, motivated, and qualified social work practitioner to join our team. You need strong interpersonal and organisational skills. If you are a passionate practitioner who is committed to making a positive difference to the lives of our service users, you will find lots of job satisfaction being a Futures+ social worker. The role can be demanding so you need to be a team player with a can-do attitude.
Within Futures+, we have two hubs, Preparing for Adulthood (PfA) and Community Inclusion (CIH). We are recruiting an experienced full-time 37 hours a week social worker for CIH. PfA Hub supports children aged 14 or above, who have complex needs, transitioning from children services to adult services. Service users will transfer from PfA hub to CIH hub either when their education ends or when they turn twenty-five years old. There is no upper age-limit for CIH.
In Futures+, we embed a person-centred and strength-based approach when working with our service users, their carers and/or families to carry out care act assessments, formulate care and support plans, put together panel applications and conduct reviews. We work collaboratively with colleagues from other agencies and contribute towards multi-agency meetings. For a few days a month, you will take up the role as a duty officer answering enquiries and dealing with other urgent requests from the public or other professionals. Please be assured that when you are on duty, you will be supported by a senior social worker and work alongside another team member. In Futures+, we pride ourselves in our strong team spirit. We have an induction program to support new members of staff.
You must be a registered social worker. You must be able to demonstrate sound knowledge of relevant legislation and guidance. This includes the Care Act 2014, the Mental Capacity Act 2005 and the Human Rights Act 1998. You must also have a sound knowledge about person-centred and strength-based approaches. Collaborative working is an important part of our work; therefore, you are expected to demonstrate experience in multi-agency working.
Depending on your experience, you will be expected to undertake supervisions of Enabling and Transitioning Officers.
