The Senior Primary Care Medicines Management technician is an integral part of the Community Health and Wellbeing Team (CHWT) and supports the pharmacist to manage patient medication records to limit waste and resynchronize medications. Working on the advice of the CHWT Pharmacist, the post holder will attend weekly MDT meetings and undertake home visits to take drug histories and counsel patients on medications. The post holder will liaise with other Community Pharmacy Services to ensure patients are supported with their medication administration at home.
The post holder will work within a range of clearly defined occupational policies and:
1. Ensure a safe, evidence-based, and cost-effective medicines management service is provided by pharmacy technicians to areas within Cwm Taf Morgannwg University Health Board (CTMUHB).
2. Support multi-disciplinary working and the development of Medicines Management Services.
3. Develop, implement, and monitor standards for Medicines Management Services delivered by Pharmacy Technicians and Pharmacy Support Staff within CTMUHB.
4. Support the delivery of the Cash Releasing Efficiency Savings (CRES) Schemes.
5. Support the quality and safety requirements in relation to Medicines Management across the locality.
6. Audit and monitor Primary Care Medicines Management Services provided by Pharmacy Technical Staff within CTMUHB.
7. Act as a role model and lead the training and development for Pharmacy Technicians providing Primary Care Medicines Management Services.
Facilitate the Primary Care Teams in Medicines Management and safety projects identified through objective setting and implementing and influencing change.

The post holder will:
1. Take forward the national campaigns by acting as a focus, driving force, and facilitator to allow CTMUHB staff to develop and achieve the areas targeted at Primary Care:
* Improving Medicines Management.
* Achieving efficiency.
* Reducing harm and variation.
* Supporting the Primary Care Teams to improve the experience and outcomes involving medicines, for individual patients by:
o Addressing unmet pharmaceutical needs.
o Helping patients get the best from their medicines.
o Improving service efficiency and reducing waste.
o Improving access to Medicines Management Services.
1. Lead and coordinate activities on agreed areas within the prescribing team and CTMUHB, relating specifically to the Medicines Management (MM) and patient safety.
2. Work closely with strategic partners of the Local Health Board, Primary Care Clinicians, GP Practices, other CTMUHB multi-disciplinary staff, Community Pharmacists, Patients, and Voluntary Groups.
Managerial Responsibilities:
1. Responsible for development and management of, and assisting in the delivery of medicines management services by Pharmacy Technicians.
2. Support Clinical Pharmacists and Nursing staff providing Medicines Management Services to designated clinical area(s) in accordance with Professional Standards, Departmental and Health Board Policies and Procedures.
3. Work closely with Specialist Pharmacists and other Healthcare Professionals in developing Primary Care Pharmacy Services.
4. Write and develop protocols, standard operating procedures, and service training packages for Primary Care Pharmacy Technical Staff.
5. Review and update all Primary Care Medicines Management Technician procedures, and monitor compliance.
6. Collaborate with the Specialist Pharmacists in the organization and monitoring of Medicines Management Services including data collection, audit, and presentation.
7. Communicate developments in Medicines Management Services to all Pharmacy Department support staff through written communication and departmental meetings.
8. Plan and develop Primary Care Technician work plans alongside other staff on a regular basis.
9. Work with Senior Pharmacy Staff (training and development) for planning and delivering technician training.
10. Manage, develop and motivate staff and ensure the cooperation of all staff allocated to the Medicines Management Service.
11. Represent Primary Care Technicians at departmental and University Health Board wide meetings as appropriate.
12. Attend regular Primary Care Pharmacy management meetings.
13. Lead on recruitment interviews and selection for Pharmacy Technician staff to provide Medicines Management Services.
14. Appraise Pharmacy Technicians who provide Medicines Management Services.
15. Participate in regular reviews with pharmacy colleagues, to improve the quality and efficiency of Services.
Medicines Management Services:
1. Support Clinical Pharmacists and Nursing staff to provide Medicines Management Services in Primary Care, in accordance with professional standards, departmental and Health Board policies.
2. Oversee the provision of Technician Medicines Management Services to designated area.
3. Identify any problems/discrepancies between patients’ own drugs and patient record, and bring to the attention of the Clinical Pharmacist or Medical Staff as appropriate.
4. Deal with enquiries from patients, Medical and Nursing staff within competence or refer to appropriate Pharmacy Staff if necessary.
Education, Training, Research and Development:
1. Provide, organize, and plan (in cooperation with the Senior Pharmacy Technicians and Pharmacists) Medicines Management training for Pharmacy Technicians.
2. Identify and address training needs of Pharmacy Technicians.
3. Produce in-house training packages for Pharmacy Technicians providing Medicines Management Services.
4. Participate in the training schemes and tutorials arranged for Pharmacy Technicians and Nursing staff within CTMUHB as appropriate.
5. Provide teaching and/or presentation sessions for Pharmacy Technicians.
6. Undertake and supervise audit and practice research for Medicines Management Services.
7. Participate in continuing professional development in accordance with National Guidelines from the Royal Pharmaceutical Society/General Pharmaceutical Council.
8. Participate in induction of pharmacy staff, assist with departmental visits and short-term placements for potential recruits to the Service.
9. Assist in resolving prescription problems in consultation with Medical and Nursing staff as appropriate.
10. Work at all times in accordance with Departmental Standard Operating Procedures, current Legislation and National Guidance.
To design and implement a local action plan to ensure the continuation of the work and ideas from the CTMUHB Medicines Management Strategy are spread within CTMUHB and to the wider health community, such as:
1. The development and implementation of national and local Medicines Management Strategies which promote improved medicines management and patient safety, identified in collaboration with the GP practice partners, staff, and the CTMUHB prescribing team.
2. Promoting improved education on Medicines Management within the CTMUHB and across the wider health community via local networking including patient participation groups and forums, care homes, such as public health campaigns and production of patient information leaflets.
3. To help identify and lead on the development of local projects and initiatives related to Medicines Management and patient safety. This will utilize the IQT methodology to test, research, and develop improvements in medicines management and clinical care.
4. To design and create forms, IT templates, posters, patient information leaflets, and reference documents.
5. To identify and manage projects for improvement of patient safety within GP practices or other primary care contractors e.g. anticoagulation.
6. To support the development and implementation of a medication review strategy that improves patient outcomes and reduces harm.
7. To be aware of medicines management issues within the primary care contracts and local enhanced services e.g. GP, community pharmacy, dental etc., and how MM projects would support their implementation within primary care.
Communication:
1. Cascade educational material to members of the Primary Healthcare Team and support staff including the Health Board prescribing newsletters.
2. Preparing written and verbal reports for practice staff including General Practitioner, Practice Nurses, Health Board and support staff at an appropriate level. Collate and analyze Practice prescribing data, providing appropriate feedback to the Practice regarding activity.
3. Obtaining drug, therapeutic, and pharmaceutical information for Practices when required.
4. Communicating with Practice staff and patients the reasons behind why agreed therapeutic changes are being implemented by letter, telephone, or by consultation.
Patient Client Care:
1. Undertake patient medication reviews from clinical records for groups of patients agreed by the practice. This process includes the following responsibilities:
* Identifying patients for potential medication changes in specific therapeutic areas.
* Carrying out an initial assessment of the patient’s medication and arranging for any necessary drug monitoring.
* Identifying any potential problems for referral to the GP.
* Documenting any recommendations as a result of the medication review, discussing these with a GP as appropriate.
* Implementing any agreed changes.
* Communicating any changes to the patient and/or carer, and relevant Health Care Professionals e.g. Community Pharmacist.
* Ensuring any necessary follow-up as a result of the medication review e.g. checking results of blood tests, are carried out.
* When necessary, see patients to discuss changes and/or patient concerns regarding treatment changes following authorization from a GP.
* Provide a patient counseling service by telephone or by consultation.
* Provide both written and verbal patient information, medication review, and if necessary supply information to the patients about changes in their medication.
Financial and Physical Resources:
1. Monitors practice prescribing against the medicines management local enhanced service to enhance clinical effectiveness on behalf of the Health Board.
2. Advises practice on prescribing expenditure and implementing plans to reduce costs where appropriate without detriment to patient care. Implements plans to facilitate changes in prescribing.
3. Ability to reconcile medication from two or more sources to support GPs in discharge procedures.
