Job summary To provide high quality patient-centred care and specialist rehabilitation to adults referred to the Rehabilitation Service.Participate in the development of the Rehabilitation Service in the community and within the Community hospital bedded environments, where there are teams supporting hospital inpatient unitsTo provide advice regarding the rehabilitation management of patients to families, carers and other members of the MDT.Responsibility for a caseload of patients within both the community and/or the community hospital bedded environment.Carry out duties autonomously but with the ability to seek support and informal and formal supervision from Senior Therapists within the teams.Alongside Senior clinical staff and Clinical Educator, give support, guidance and training to Rehabilitation Assistants, Assistant Practitioners, Apprentices and Students on placement within the team.
