Job summary The Post holder will be responsible for investigating patient pathways in accordance with national and local guidance, and validation of data used to measure the Trust's compliance with Referral to Treatment (RTT) national returns for admitted, non-admitted and incomplete performance. They will provide timely information to Trust managers are to ensure national targets are not breached.The post holder will work to ensure completeness and quality of data, using all networks and resources to expedite efficient patient pathways. The post holder will support the management of referrals through the E-RS system. They will provide clear guidance and training to all staff on effective validation of RTT data and complete audits as part of a continuous cycle for improvement in data quality and pathway monitoring. Main duties of the job To assist operational managers in the validation and performance management of PTL monitoring, ensuring booking, data recording, clinic and elective admissions outcomes of the patient pathways are proactively managed in order to achieve 18-week targets.To ensure that all patients are tracked effectively throughout their 18-week episode of care and that any problems and blockages are identified in a timely way for remedial actions that ensure patients don't breach their 18 Weeks and other Access targets and KPIs. To validate patient pathway reports in time to meet the weekly and monthly timescale for the submission of RTT data.Where appropriate to liaise with other departments and external providers for patients being referred as an onward/tertiary referral ensuring the patients journey is planned booked and coordinated within a specified time limit.To routinely monitor and validate the open patient pathways - including but not limited to Outpatient Waiting list, Inpatient Waiting Lists, RTT Incompletewaiting list - for inaccuracies, inconsistent and incomplete data and amend accordingly.To highlight potential breaches to patient pathway coordinators/booking and admissions staff in order to bring forward appointment/admission dates.To support the management of the Trust e-RS system, and effective processing of referrals received through the system.
