Job summary We are looking to recruit an enthusiastic, Patient Flow Co-ordinator, within the North & East iToCH based in Carew House. This person requires previous experience in patient flow. We are looking for an individual who can demonstrate attention to detail and a high level of diligence towards the patients and their health care journey. Main duties of the job The role of the iToCH is to receive referrals for services from around the system, assess the referrals, prioritise them and integrate health, social care and the voluntary sector responses in a timely way. The team are key in maintaining the flow of people who are being discharged from bedded care settings, coordinating support for these people and also coordinating support for people who are in their home setting. The aim is to have the right care, in the right place, at the right time with outcomes that improve health and well-being. In this role you will work closely with the North & East iTOCH Team, the Head of Patient Flow and will need to understand the level of capacity, demand and escalation across various parts of the health and social care system. You will be accessing the different recording and referral systems, inputting, and relaying information, recording and reporting data and presenting reports in clear and understandable formats. You will be communicating with people throughout the system on a daily basis. You will have an important role in supporting and teaching the administrative, assessors and other staff with how to access and operate the different recording and referral systems in a changeable and developing way of working.
