Main duties of the job
To provide administrative support to the Doctors and Health Professionals at the surgery and a coordinated approach for patients.
Purpose of job/responsibility:
Coordination, recall and booking for the following as required:
* Frailty / Advance Care Planning
* Learning Disability reviews
* QoF recalls
* Dementia reviews
* Post natal and baby checks
* Locally enhanced services monitoring
* PCN DES and IIF targets
* Liaising with patients, hospitals, social services and other outside agencies.
* Liaison between the clinical team, patient and hospital specialists in order to coordinate outpatient and community care.
* Lead on nursing homes and be their first point of contact with the practice.
Assistance with general administrative tasks including:
* Scanning and document processing
* Repeat prescription processing
* Appointment booking
* Vaccination schedule monitoring and planning
* Gathering statistical information as required.
