Main duties of the job
Health and Wellbeing coaches empower people to manage their own health and improve their health outcomes. Based on what is important to each person, coaches help to improve a person's knowledge, skills and confidence in managing their health, building on their strengths and supporting them to make changes that impact their health and wellbeing. Health and Wellbeing Coaches provide both one-to-one coaching and group sessions.
You will be enthusiastic about improving outcomes for ourpatients, and have experience in using a coaching methods or approaches such asmotivational interviewing to help people improve their health and wellbeing. Youwill have a firm belief that people have untapped resources within them, thatcan be unleashed by providing high quality, non-judgemental support.
Excellent communication skills, relationship-building, andpatient-focus are essential to the success of this role. You will be will beproactive with a flexible attitude, keen to work as part of a team and keen todevelop effective and enduring relationships between Primary Care and partner organisations.
Therole involves working directly with patients to co-create health and wellbeingplans that are individualised, holistic, and realistic, and use a coachingapproach to achieve goals. The Coach will work with Practice colleagues andother community organisations, and will help identify areas for improvement inservices.

Ely Primary Care Network (PCN) are a collaborative group of six of the eight GP Practices in East Cambridgeshire.
We are considered an innovative and high-achieving PCN, and have a track record of making a positive difference for our patients and partners.
Ely PCNs is a key part of a wider network of health and social care organisations in East Cambs and together we are having real success supporting our local population with their health, wellbeing and social needs.
Our PCN employs 30+ staff including Social Prescribers, Pharmacists, Physios Care Coordinators and Coaches.
We currently have six Health and Wellbeing Coaches in our team. The team is supported by Vandana Manocha, our Lead Health and Wellbeing Coach. All Coaches have access to ongoing skills development, support, and supervision with Vandana and GPs and able to build their skills and experience within the role.
The post-holder will have a permanent contract of employment with one of our six Practices.

Job responsibilities
The Health and Wellbeing Coach is a key role, its aim being to help improve health outcomes of the patients who live in the East Cambridgeshire PCN catchment area. They will work with the three GP Practices in Ely North PCN, the PCN Clinical Directors, other Health and Wellbeing Coaches, Social Prescribers, Care Co-ordinators, and secondary health and social care services.
The health and wellbeing coach will work closely with patients of low to medium complexity who will usually have one or more long-term conditions. The role will involve direct work with patients and their families, by phone, by video conference, or face-to-face. They will support patients to take pro-active steps to improve the way they manage their physical and mental health conditions, based on what matters to them.
The Health and Wellbeing Coach will be responsible for delivering individual and group coaching sessions. They will use coaching techniques to support people to take an active role in their health and wellbeing. The coach will play a critical role in engaging and motivating patients and providing access to interventions such as self-management education and peer support. The coach will manage and prioritise a caseload, according to the needs, priorities and support required by individuals in the caseload.

The post holder will work closely with the other members of the primary care team and multi-disciplinary care teams in the management/decision making about care and service provision for individual patients. This will include:

The Health and Wellbeing Coach responsibilities include but are not limited to the following:

8. Provide one-to-one and group health coaching support for people with one or more long-term conditions, based on what is important to them, with the aim of: improving peoples knowledge, skills, confidence in managing their condition/s, empowering people to manage their own health and improve their health outcomes and support them in making changes in their health related behaviour
9. Manage and prioritise a caseload, in accordance with the needs, priorities and support required by individuals in the caseload. It is vital that you have a strong awareness and understanding of when it is appropriate or necessary to refer people back to other health professionals/agencies, when what the person needs is beyond the scope of the health and wellbeing coach role when there is a mental health need requiring the patient to be referred to an appropriately qualified practitioner.
10. Work as part of a multidisciplinary multi-agency team to promote health coaching and to be ambassadors for Personalised Care and Supported Self-Management, modelling the coaching approach in their work.
11. Ensure that GPs, practice nurses, practice pharmacists and other members of the primary care team understand the Health and Wellbeing Coach role, how to refer to them, and which patients may particularly benefit from health coaching.
12. Support local health, social care and voluntary sector professionals to make appropriate referrals to the service. Promote and raise awareness of the health coaching service particularly to groups and communities that experience barriers to access.
13. Attend and contribute to team, practice, and PCN meetings and events as required by the service.
14. Work flexibly, adapting to the needs of the service and client group while maintaining the integrity of the role.
15. Participate in regular health coaching supervision and continual learning. This may include, but is not limited to, any or all the following: regular contact with service supervisor ;refresher training sessions; buddying with peers; peer support sessions; 1:1 support from a practitioner with more health coaching experience; Action Learning Sets; e-learning to revisit or deepen training; On-going improvements to systems and processes.
16. Collect service user experience and impact of health coaching as part of the delivery of personalised care. Participate and collect information that measures the impact of health coaching as an intervention that supports embedding personalised care into local health systems.

23. Promote health coaching, its role in supported self-management as a part of personalised care, in addressing health inequalities and the wider determinants of health;
24. As part of the PCN multidisciplinary team, build relationships with staff in GP practices within the local PCN, attending relevant multidisciplinary meetings, giving information and feedback on health coaching.
25. Be proactive in developing strong links with all local organisations to encourage referrals, recognising what they need to be confident in the service to make appropriate referrals;
26. Work in partnership with local agencies to raise awareness of health coaching and how improving peoples knowledge, confidence, skills can enable them to improve their ability to manage their long-term conditions and reduce reliance on clinical services;
27. Provide referral organisations with regular updates about health coaching, including information on how to encourage appropriate referrals;
28. Seek regular feedback about the quality of service and impact of health coaching on referral agencies;
29. Be proactive in encouraging equality and inclusion and case-finding, through self-referrals and connecting with all diverse local communities, particularly those communities that statutory bodies may find hard to reach.

33. Work closely within the multidisciplinary team and with GP practices within the PCN to ensure that the relevant SNOMED codes to record activity are inputted into clinical systems (as outlined in the Network Contract DES), adhering to data protection legislation and data sharing agreements.
