Rochford District Primary Care Network (PCN) have a fantastic opportunity for a permanent data and outreach coordinator to join our team. This position is offered on a full-time basis, working 37.5 hours per week, ideally Monday to Friday and is covering the following practices: Ashingdon Medical Centre The Practice Leecon Way The Puzey Family Practice Wakering Medical Centre The role involves providing high quality, comprehensive and effective administration skills, whilst providing a central co-ordination role for patient care planning across member practices. Key aspects of the job include: Minute taking, meeting organisation, and following up with actions. Producing data reports and audits from SystmOne. Acting as the first point of contact for the clinical team to provide support with general queries. Maintaining the staff’s clinical rotas on SystmOne. Providing administration support to the PCN educational events. Working with the clinical team with scheduling outreach events and forums. Liaising with patients and members of the community. You will need to be confident individual who can communicate with a ride range of professionals at all levels. The successful applicant will have the ability to work without supervision using their own initiative, organising and prioritising own and other workloads to changing and often tight deadlines.
