Specialist Practice Nurse with experience in Diabetes
An exciting opportunity has arisen for an experienced and enthusiastic Specialist Practice Nurse to join our forward-thinking team at Riverside Health Partnership.
The successful candidate will work to ensure a high level of care is delivered to our patients. The practice is very supportive of any development opportunities, but is looking for a dedicated nurse that can really hit the ground running.

Riverside Health Partnership serves over 23,000 patients in Bassetlaw. We are a busy 4-site practice with a team of 13 GPs, 6 ANPs, an excellent team of Practice Nurses and Health Care Assistants.
We are a consistently high performing GP training practice operating on SystmOne clinical system. The successful applicant must have their own transport as working across four sites may be necessary.
Main duties of the job
1. Able to contribute to the provision of a full and comprehensive nursing service to patients of Riverside, with particular lead responsibility for diabetic patients under the guidance of our GP specialists in this field.
2. Able to provide appropriate preventative health care and advice.
3. Participate in clinical governance activity and contribute to the improvement in quality of health outcomes through the practices audit programme.
4. Clinically related administrative and non-clinical duties as necessary.
5. Engage in appropriate educational activity in order to maintain clinical competence and performance, including the completion of objectives agreed by yourself and the practice in an annual Personal Development Plan.
6. Participate in the training and development of nurses in the practice, when appropriate.

Person Specification
Experience
* Registered Nurse (RN) with valid NMC (Nursing and Midwifery Council) registration.
* Specialist diabetes qualification, such as a Diabetes Diploma or equivalent.
* Proven experience in managing patients with diabetes in a primary care setting.
* Strong clinical skills, including the ability to assess, plan, and deliver care for diabetic patients.
* Excellent communication and interpersonal skills, with the ability to build rapport with patients and colleagues.
* Knowledge of current national guidelines and best practices for diabetes management.
* Ability to work autonomously and as part of a multidisciplinary team.
* Experience working in a GP practice or primary care setting.
* Experience in delivering structured education for diabetic patients.
* Holds a nurse prescribing qualification (V150 or V300).
* Experience with data management systems such as Systm One.

£45,000 to £50,000 a year Equivalent Band 6 / Band 7 dependent on skills and experience.
