The post holder will work alongside an existing consultant psychiatrist/medical psychotherapist (8PAs) to provide consultation for and liaison with GP practices and neighbourhood practitioners across Stockport using a collaborative care model with patients 16 years and above. This service user group have high levels of complexity who may be at risk of harm, with complex poly-pharmacy and where diagnosis/formulation is needed.The post holder will liaise with Primary Care clinicians and managers and Primary (e.g. IAPT) and Secondary Care Mental Health Services (such as psychiatrists, physiciancolleagues and substance misuse services) amongst others. There will also be liaison with clinicians working in primary care, GPs, practice nursing staff and community healthcare staff.This will involve identifying the learning needs of Primary Care and Primary Care Mental Health staff and others and developing a suitable educational and training approach to be delivered in a variety of formats and settings such as formal direct teaching, workshops and providing a space for reflection and discussion. Supervision of primary care staff, including GPs and IAPT staff is key feature. The post holder will also be expected to engage actively in education and training of the services own staff including liaison practitioners. The service has a GP plus post attached from a local GPs in deprivation training scheme, and the post holder will be encouraged to be a clinical supervisor.The post holder will attend relevant project planning meetings as well as other PennineCare and CCG/ Stockport Together events that support the delivery of the project.The post holder will take an active role in developing this area of work with the support of the team lead, rest of the team, IAPT services, the Psychological Medicine Service,Pennine Care and local commissioners.Clinician and patient rated outcome measures, patient rated experience measures and health care utilization data will be collected. The amount and impact of educational activity will be evaluated. This will be done with the support of existing Pennine Care and CCG structures and externally commissioned providers. The post holder is expected to take an active role in supporting and facilitating this.
