Detailed job description and main responsibilities
The post holder will be one of four Consultants providing acute care to patients under the Crisis Resolution Intensive Support Service (CRISS). Although a city-wide service, the post holder will work primarily from the West area of the city, with a focus on facilitating early discharge from acute wards and overseeing the progression of patients through the home-based treatment pathway as an alternative to hospital admission via intensive support. The Consultant will also play a key role in supporting and overseeing crisis and extended assessments in the community, working collaboratively with the multidisciplinary team (MDT) to formulate recovery-focused care plans.
Although working primarily from the West base, the post holder will be expected to work flexibly when needed and when appropriate, for example assisting colleagues in other areas of the city when demand has outweighed resource, or if prior knowledge of a particular patient on the Crisis Assessment Unit or in the s136 suite means the post holder is the most appropriate person to see that individual.
