Act as the point of contact for patients accessing the service arranging appointments by phone or in person as required Develop a comprehensive knowledge of wider support services in the local and city wide area and support individuals to access those that are relevant to their needs and work on what matters to them Coordinate the referral process from start to finish, communicating regularly with all stakeholders Record monitoring data systematically on clinical systems, produce timely reports and contribute to the overall evaluation Capture key information and gather feedback on the impact of social prescribing on health and wellbeing Attend reflective practice and/or clinical supervision, team meetings and line management meetings as required Build strong working relationships with internal and external services in both the statutory and voluntary sectors To promote the link worker/social prescribing model Support the other Social Prescribing Link Workers; providing advice, development support, motivation and encouragement Provide professional peer support to the SPLWs around complex cases Undertake annual staff appraisals and bi-monthly supervisions Lead on project meetings to update around work undertaken, feedback around unmet needs the team may be finding, what may need development, what is working well and what are the challenges etc. Be the safeguarding lead for the Social Prescribing Link Worker team Create and maintain referral routes into the scheme via a wide range of frontline professionals in the health, social care and community development sectors Give patients a voice, ensuring they influence operational and strategic direction of the service. Be an active and contributing staff member, attending meetings as required and working constructively as part of a team to meet all organisational and project goals Attend mandatory training and abide by all organisational policies and procedures, including Safeguarding & Child Protection, Health and Safety and Equality and Diversity Attend appropriate continuing professional development, training and networking events This is not an exhaustive list of duties and may be subject to change due to organisational priorities, funding and as agreed by management, in consultation with the post holder
