Job overview
The post holder will work as part of the children’s community health commissioned service under the direction and supervision of the SCPHN Health Visitor/School Nurse/ Family Health SCPHN/ Staff Nurse, contributing to the provision of high quality services for children, young people, parents, carers, and families working within the framework of the Healthy Child Programme and Best Start In Life priorities. Being pro-active in delivering the national and Public Health agendas in response to identified health and social needs, working in partnership with other agencies to meet agreed standards, objectives, and statutory requirements.
Main duties of the job
1. Promote the needs of children with SEND, young people with SEND, and their families in the development of community child health services.
2. Support the SCPHN Health Visitor/School Nurse in the delivery of the Healthy Child Programme, ensuring services are provided within the tiered model approach – universal, universal plus and universal partnership plus.
3. Be professionally accountable and responsible, adhering to and applying the Trust standards.
4. Apply the principles of the Solihull approach in all work undertaken with children, young people, and families.
5. Apply the Best Start in Life principles where applicable.
6. Work within family homes delivering packages of care to support families in addressing identified health and development needs.

Detailed job description and main responsibilities
This post involves working with families of school-age children who have additional needs. The post holder must hold a current UK driving licence.
1. Implement planned programmes of care following assessment by SCPHN/Staff Nurse.
2. Promote the needs of children with SEND, young people with SEND, and their families in the development of community child health services.
3. Work in partnership with parents and carers and partner agencies in a variety of settings to help them create an environment where children and young people can develop and learn.
4. Work in partnership with parents and carers to build on their skills so that children’s and young people’s wellbeing, resilience, and life opportunities are promoted.
5. Establish and maintain positive interpersonal relationships with other team members characterized by open communication, trust, and respect.
6. Promote effective team working, sharing knowledge and information, encouraging a supportive environment to promote good working practices and outcomes.
7. Provide a standardised, consistent approach (applying the evidence base) to the identified, individual needs of the child, young person, and family.
8. Contribute to the development, delivery, and reporting of health promoting activities within a variety of settings.
9. Deliver evidence-based early intervention programmes to support children, young people, and families e.g., speech and language development, prevention of accidents in the home, promotion of oral health, promotion of healthy weight, and physical activity.
10. Signpost and provide information for parents and young people, communicating information in an understandable form to parents, children, and young people.
11. Work flexibly by undertaking any other duties (within scope of practice) compatible with the post as directed by the management team SCPHN/Staff Nurse in relation to service needs.
Person specification
* NVQ Level 3 Qualification or equivalent.
* Experience of working with and supporting parents and families.
* Provide evidence-based childcare advice and guidance to parents/carers regarding care needs, learning, safety, play, and therapy programmes.
* Excellent verbal and communication skills with children and parents.
* Ability to work as part of a team.
* The post holder must have a full UK driving licence and be able to make their own travel arrangements for business usage.
* Administrative and IT Skills including Word.
* Ability to engage positively with service users and colleagues.
* Child Care Qualification.
* Experience working with a range of statutory and voluntary organisations.
* Evidence of family support in a community setting.
* Knowledge of the Solihull Approach.
