We are looking for a dynamic and enthusiastic team player within the Colorectal department as a Colorectal PCFU Coordinator.
You will provide a timely, safe and appropriate clinical cancer surveillance programme tailored to the individual's cancer staging.
Individual holistic needs will be addressed within the surveillance programme through care planning, signposting, education and personalised information, facilitating individuals to self-manage their own health going forwards.
End of treatment summaries and individualised surveillance programmes will be provided to the individual and primary care team to promote seamless transfer of care and facilitate cancer care reviews in the community.
Appropriate re-access pathways are in place in the event of abnormal results or changes within the individual's recovery.
This position is Fixed Term until 31st March 2025, current ESNEFT Employees will be offered on a Secondment basis.
• Responsible for ensuring that all assessments, interventions and outcomes are documented on the Somerset cancer registry.
• You are responsible for monitoring and tracking the individual’s on-going surveillance programme and the investigations results within it.
• You will liaise with the nurse consultant to ensure that the next investigations have been requested and scheduled.
• Abnormal results should be highlighted to the Nurse consultant in a timely fashion.
• Delays in investigations should be highlighted to the responsible nurse consultant and escalated to management as required.
• Answer calls to the OAFU telephone line and escalate issues or concerns and when to seek additional support/guidance from the CNS team.
• Send out template results letters.
