The post holder will, under appropriate supervision, provide the first point of contact for patients presenting with undifferentiated, undiagnosed problems, utilising history taking, physical examination, problem solving, and clinical decision-making skills to establish a working diagnosis and management plan in conjunction with patients. The post holder will undertake both morning and afternoon clinics within various GP sites within the PCN, as well as home visits and administrative work.
Key Responsibilities:
1. Participate in treatment and preventative healthcare.
2. Provide direct clinical care to patients using established clinical guidelines.
3. Consult and refer patients to physicians, medical specialists, and other healthcare professionals.
4. Provide one-to-one consultations to patients, both in General Practice and patients' homes, taking medical histories, performing physical examinations, analysing the consultation, diagnosing, and explaining the medical problems.
5. Recommend and explain appropriate diagnostic tests and treatment.
6. Request and interpret results of laboratory investigations when necessary.
7. Perform specialized diagnostic physical exams and treatment procedures.
8. Provide patient education in preventative healthcare.
9. Conduct telephone consultations which may involve discussing the results and implications of laboratory investigations with patients as well as dealing with routine and emergency healthcare conditions.
10. Request laboratory tests as required and agreed under supervising GP.
11. Update patient records in accordance with practice policies and procedures.
12. Contribute to the practice systems to manage common chronic medical conditions.
13. Ensure observance and adherence to the Health and Safety at Work Act 1974.
14. Work to the agreed standards, policies, procedures, and protocols of the Practice and be fully conversant with emergency procedures.
15. Carry out other duties as directed by the line manager/Practice Manager which are consistent with the duties and responsibilities of the post.
