Hours: Full-time, 5 days over 7, 09:00 - 20:00
Service Care Solutions is actively recruiting for a Band 6 Senior Nurse Co-ordinator to join the NHS on a temporary contract. This is a fantastic opportunity for a skilled nurse with the right qualifications and experience to make a significant impact in patient care.
Key Requirements:
1. Driving License and Access to a Vehicle: Essential.
2. Experience in District Nursing: Essential.
3. Physical Health Assessment Module for Deteriorating Patients: Essential.
Key Responsibilities:
1. Triage Referrals: Manage referrals for the Discharge to Assess pathway, ensuring alignment with referral criteria.
2. Comprehensive Assessments: Conduct holistic assessments, planning, implementation, and evaluation of care for patients, focusing on areas such as Chronic Disease Management, Tissue Viability, and Palliative Care.
3. Public Health Initiatives: Deliver health promotional programs to improve overall health and reduce inequalities.
4. Service Delivery and Accountability: Lead service delivery with a focus on quality, ensuring the best outcomes for clients/patients.
5. Rehabilitation Plans: Assess, plan, and implement tailored rehabilitation programs for patients with complex needs, working collaboratively with patients, carers, and other agencies.
6. Interdisciplinary Coordination: Establish and maintain effective relationships with health and social care professionals, as well as statutory and voluntary organisations.
7. Education and Empowerment: Provide patients and relatives with relevant information and education, promoting meaningful choices, dignity, and independence.
8. Records Management: Maintain accurate, contemporaneous written and computerised records.
Qualifications and Professional Development:
1. Further knowledge obtained through accredited courses, workshops, and training programs is essential.
2. Confidence in recognising personal limitations and seeking advice when necessary.
