LD and SEND Families Social prescribing is a way of engaging patients and their families in primary care with a resource which provides support within the local community.
In addition, it provides GPs with a non-medical referral option that can align to existing treatments to improve health and wellbeing.
The role will provide information and support to patients and their families in addition to becoming the link between the patient, GP and other service providers. The role will require managing and prioritising your own caseload, in accordance with the needs, priorities and any urgent support required by individuals.
Main duties of the job
* Receiving and actioning referrals from GPs, Special Educational Needs Co-ordinators (SENCOs) and wider adult social prescribing teams. This list may vary depending on need.
* Providing personalised support to individuals, their families, and carers to enable them to take control of their well-being and reducing health inequalities.
* Develop trusting relationships by giving people time and focus on what matters to them.
* Taking a holistic approach, based on the persons priorities and the wider determinants of health.
* To co-produce a personalised support plan to improve health and wellbeing, to introduce or reconnect people to community groups and statutory services.
* It is vital that the Social Prescribing Link Worker has a strong awareness and understanding of when it is appropriate or necessary to refer people back to other health professionals or agencies.
* Ensure patients and their families are supported and have awareness of safeguarding processes for vulnerable individuals and can provide opportunities for the person to develop friendships, a sense of belonging, and build knowledge, skills, and confidence.
* Work together with all local partners by establishing and facilitating the community assets available.

Hinckley and Bosworth Medical Alliance is a GP Federation that supports 12 GP practices across the Hinckley and Bosworth locality, which form 3 Primary Care Networks.

Person Specification
Experience
* Experience of working in a similar role.
* Experience of working with vulnerable and young people.
* Experience of working in health and social care environment.
* Experience of coordinating services from diverse providers.
* Experience of working in liaison capacity with social care.
* Experience of seeing patients and carers in a practice-based setting, in their own home or in a community setting.
* Experience of working within a mental health, Learning Disability and Special Educational Needs setting.
Qualifications
* GCSE grade A to C in English and Maths.
* NVQ Level 3, Advanced Level or equivalent or working towards a commitment to professional and personal development.
