Job summary
This is an exciting opportunity to join theNimbuscare team in the role of Care Navigator, to support the Nimbuscare teamin the delivery of our new Urgent Care / Out of Hours contract.
You will support the smooth and efficient running ofthe GP Out of Hours service, in providing operational shift support to ourclinical teams working in our centres face to face or remotely as part of ourClinical Assessment Service (CAS).
You will liaise with Healthcare Professionals,patients, and other colleagues to ensure a timely and high standard of care isdelivered.
You will make and receive telephone calls and willwork in line with Standard Operating Procedures.
This role contributes to the effective running ofthe GP Out of Hours service across multiple locations (York, Scarborough,Malton, Whitby and Selby)
Main duties of the job
Toactively manage the patient queue in SystmOne to ensure optimum servicedelivery to patients referred from NHS 111
Toaction calls received from Healthcare Professionals
Tosupport the clinical team with onward referrals where appropriate
Toco-ordinate patients who need to be seen F2F and where appropriate home visits
Toprocess routine primary care referrals
Tosupport the Operational Services Team with routine workload bookingpatients into clinics for other contracts / services

Qualifications
Essential
17. Minimum level 2 Maths and English or equivalent (GCSE Grade C or Grade 4 or above)
18. Experience in working with SystmOne

21. Good numeracy, administrative and organisational skills
22. Excellent customer service skills
23. Excellent telephony skills
24. Understand and observe strict confidentiality
