Job Overview
The learning disabilities access team manages all referrals to the community learning disability services, ensuring they are effectively and efficiently screened, prioritised, and processed.
Access clinicians will ensure that a holistic core assessment is completed for all referrals accepted to the service in keeping with the standards set out within the access pathway and referral exclusion criteria. This will include identification of clinical risk, a full case history relevant to the reason for referral, and physical health baseline checks. Clinicians in the access team will review patients routinely who are waiting to be allocated for treatment on a clinical pathway and reprioritise them in response to an updated risk assessment, avoiding harm while waiting for treatment.
If necessary, the Access Team clinicians will escalate concerns and arrange a multi-agency meeting (MAM) to mitigate risks and avoid deterioration of mental and/or physical health, taking urgent and assertive action to avoid admission when necessary.
Successful applicants must hold a professional registration and a driving license.

We are a busy dynamic team who are the first point of contact for patients, families, carers, and referrers of those accessing our multidisciplinary community team.
We have an exciting opportunity to recruit two enthusiastic LD Access Practitioners into our Community Access Team. These positions are open to qualified Occupational Therapists, Physiotherapists, Speech and Language therapists, and Nurses. The role provides opportunities to develop skills and knowledge in specialist learning disability, mentoring students, clinical pathways, and multi-disciplinary work while being part of a supportive team. We will provide regular clinical and peer supervision and support you to continue your professional development.
The Access Team manages all referrals to the community adult learning disability service, ensuring that referrals are effectively and efficiently screened, prioritised, and processed. Following this, in keeping with the standards set out in the access pathway, the Access Clinicians will carry out a thorough holistic assessment of the patient. This will include identification of clinical risk, a full case history relevant to the reason for referral, and physical health baseline checks.
The Community Learning Disability Team is a Multi-Disciplinary Team, working alongside OTs, Physiotherapists, Speech and Language Therapists, Psychologists, and Psychiatrists. We also work closely with our system-wide partners in primary care, adult social care, and private and voluntary care providers to identify holistic needs, reduce health inequalities, manage and mitigate identified risks, and enable our patients to live their lives as independently as possible in the community with the right support.
The post holder will be responsible for:
* Screening referrals to the learning disability community team to support timely clinical decision-making ensuring the service referral criteria are met.
* Prioritising referrals using the threshold assessment grid (TAG), including all required risk assessment tools.
* Undertaking core assessments in person, by telephone, or virtually to identify individuals’ health needs, agree on an initial intervention plan with patients and/or carers, and allocate to the appropriate clinical pathway; formalising the plan with the multidisciplinary team where necessary to secure highly specialised advice.
* Supporting the referral of individuals to the Risk of Admission Register (ROAR) and Dynamic Support pathway (DSP) when required.
* Working collaboratively with colleagues in the access team to ensure stages in the access pathway are appropriately resourced and supported.
* Providing information, support, and guidance, and being a point of contact for service users, their carers, and the referrer to ensure timely response in changes to clinical presentation and to support health and wellbeing.
* Undertaking regular waiting list reviews (virtually or by telephone) in line with the Access pathway for all patients waiting for treatment, identifying, escalating, and mitigating emerging risks as required in order to avoid harm to patients while waiting for treatment.
* Where required, the Access Team clinicians will escalate concerns and arrange a multi-agency meeting (MAM) to mitigate risks and avoid deterioration of mental and/or physical health, taking urgent and assertive action to avoid admission to hospital when necessary.
All posts are full-time, and the usual working pattern is Mon-Fri 8.30-4.30. We encourage part-time working with a minimum of 0.6 WTE. Flexible working arrangements will be considered in line with LPT policy.
You will need a full driving licence and access to your own car. LPT operates two car schemes: Car Leasing and Car Salary Sacrifice, subject to meeting the acceptance criteria. If you currently have a lease car through the NHS, we may be able to transfer the agreement over.
