As a Clinical Pharmacist, you will be part of a multidisciplinary team to develop and run processes for repeat prescription reauthorisation, management of medicines on transfer of care and systems for safer prescribing. With a patient facing role in managing long-term conditions; working directly with patients to assess and treat conditions, as well as promote self-care. You will be part of a group of Primary Care Network clinical pharmacists working under the supervision; training and mentoring of a Senior Clinical Pharmacist. The main responsibilities include but are not limited to: 1. Patient facing Clinical Medication Review 2. Patient facing Long-term condition clinics 3. Patient facing Care Home and Domiciliary Clinical Medication Reviews 4. Management of common/minor/self- limiting ailments 5. Medicines support 6. Medicine information to practice staff and patients 7. Telephone medicines support 8. Unplanned hospital admissions 9. Management of medicines at discharge from hospital 10. Signposting 11.
