Job summary Are you a Registered Nurse with management experience? St Margaret's Hospice Care has a fantastic opportunity for a Senior Access Team Practitioner to join our Access team. Based in our Central Referral Centre, you will manage phone inquiries from patients and health care professionals to our Advice Line, triaging referrals and responding as appropriate. You will play a vital role in the transformation and development of an Access team that is clinician led. This role is based at our Hospice site in Taunton. This is a full time, permanent role working 37.5 hours per week. In return, you will receive a competitive salary of £37,338 - £44,962 per annum, depending on experience. This role offers a working pattern of 7.5-hour shifts, over 7 days per week on a rota system. As this is a role supporting transformation unsocial hours may be introduced later so to begin with supporting the working week will be a priority. Main duties of the job Part of theleadership team, you will help drive forward a revised model of working, focusing on our referral, triage and advice line. You will work closely with the Head of community services, our inpatient unit, existing Central Referral Centre and the wider hospice and external services. Base at our Taunton Hospice Hub, your day to day job will involve: Providing day to day management to the Access Team,leading on recruitment, team development, appraisal and off duty. Triaging new referrals,signposting appropriately based on individual needs, and liaising with other services, internal and external. Utilising your telemedicine skills to respond to advice line calls, drawing on the support of professionals and colleagues. Creatively reviewing processes and implementing change, working to constantly evaluating and measuring impact.
