LOCATION: Mold - Flintshire
HOURS: 24 Hours per week (potentially rising to 40 hours)
Core Responsibilities:
1. Delivery of a floating support service, conducting continuous assessments to identify the support needs of the clients and to formulate appropriate person-centred support packages.
2. Effectively manage case load and own time management.
3. Maintain up-to-date knowledge of current legislation and policies to ensure information provided to clients is appropriate and accurate.
4. Deliver housing, safety, and tenancy sustainment skills in a way that empowers and promotes young people to build resilience to manage accommodation effectively.
5. Signpost to relevant partner agencies.
6. Work collaboratively with other partner agencies.
7. Communicate safeguarding issues to relevant colleagues and statutory bodies, where appropriate.
8. Identify and record risks and formulate appropriate risk management plans, cascading any known risks or concerns to relevant persons.
9. Ensure use of good practice in risk management and lone working.
10. Respond to crisis intervention situations.
You will be based in our office in Flint; however, a full driver's licence and own vehicle is required to deliver support in the community.
General Skills and Experience:
1. Knowledge and understanding of providing support, advice, and advocacy to vulnerable clients, with the ability to communicate this knowledge.
2. Experience of working with people with complex needs.
3. Experience in delivering person-centred support within a similar setting.
4. Maintain appropriate written and electronic case management records.
5. Adhere to all Local Solutions policies.
6. Carry out other duties appropriate to this post, as necessary or as requested.
Initially, the role will be 24 hours per week (£15724.8), potentially rising to 40 hours per week (£26208) from the 30th of June.
