Acklam Medical Centre is looking for a motivated, dynamic, and enthusiastic candidate to join the management team as a Nurse Manager.
As a Nurse Manager in Primary Care you will hold diplomas in all chronic diseases such as Diabetes, Respiratory and CHD and have an overview of Learning Disabilities and Frailty.
We are a forward-thinking practice based on a single site with a practice population of 11,200 striving to provide excellent patient centred care.
Main duties of the job
Working inaccordance with the NMC Code of Conduct, the post-holder will lead anddevelop the practice of the practice nursing team in the provision ofassessment, treatment, screening, health education services and advice topatients.
Thepost-holder will develop (and ensure that the nursing team work with referenceto) practice-agreed protocols and guidelines to provide nursing treatments topatients both independently and in participation with nurse colleagues andgeneral practitioners.
The post-holder will have the required level of training and competenceand will work within patient group directions where these are available.

Job responsibilities
Management responsibilities:
1. Enable the nursing team to work effectively and provide a high-quality service to patients and to work effectively with other agencies.
2. Identify the workforce that is required to provide services to meet the needs of the patient population and negotiate with the practice management team for this workforce
3. Monitor and advise on the effective use of physical and financial resources by the practice nursing team
4. Enable the practice nursing team to apply and improve clinical governance and risk management systems
5. Lead and monitor the development and implementation of policies, standards and guidelines across the practice nursing team
6. Analyse, interpret and report data on health and well-being, related needs and service use
7. Allocate, co-ordinate, monitor and assess the work of the nursing team and individuals
8. Recruit and select staff to meet the health and well-being needs of the practice population
9. Supervise and assess staff performance and development
10. Offer and provide clinical supervision to other members of the practice nursing team and act as a clinical ''expert''
11. Develop a working environment that promotes healthy, safe and effective work outcomes
Clinical responsibilities:
12. Identify, assess and diagnose patients with complex health needs across the practice population
13. Lead the introduction and use of technical and specialist nursing treatments for patients with complex healthcare needs
14. Prescribe products to meet the needs of patients within the practice population
15. Lead and develop programmes to improve health and well-being and meet the specific needs of communities, groups and individuals
16. Work with others to change policies and improve health and well-being and reduce inequalities
17. Lead the implementation and evaluation of evidence-based practice to improve the services offered by the practice nursing team
18. Lead the team in quality assurance, development and clinical governance.
19. Work with other members of the practice team in the implementation of specific aspects of the NSFs.
20. Management of chronic disease:
21. Diagnosis, monitoring and development of individual management plans; agreeing these as appropriate with the patient and other health professionals
22. Therapeutic monitoring:
23. Checking compliance with and adherence to appropriate treatments, using an holistic patient-centred approach
24. Recognising abnormalities
25. Identifying the impact of treatment and implementing or recommending changes as appropriate.
26. Wound care and management:
27. Assessing routine wounds including trauma
28. Assessing [and prescribing] to support good wound management in complex situations ( use of Doppler technique)
29. Patient health checks:
30. Working with patients to develop a management plan where health problems or potential health problems are identified
31. Identifying health trends and public health issues and working with the team and others to combat these.
32. Risk assessment:
33. Recognising issues and gathering sufficient information to refer (eg drugs; domesticviolence; child protection; vulnerable adults; senior patients; social problems)
Health screening:
34. Providing in-depth monitoring and advice as appropriate and working with the patient in deciding on management plans
35. Considering the issues in the light of the practice population.

37. Provision of specialist and evidence based support to the team
38. Working with public health initiatives, ensuring the work of the team is up to date.
