Job overview
If you are keen to provide support to facilitate and drive the safe, effective and timely transfer of patient care from MAU to the most appropriate setting to meet the needs of the patient; then this is the role for you.
You will promote the philosophy of discharge planning as an integrated whole system approach which delivers an effective service, take active steps to support and promote flow and timely discharges whilst being proactive and diligent in ensuring timelines and workflows are upheld.
You will also actively communicate with patients and their carers with the continuous process of discharge planning and ensure they are involved at every stage, being responsible for the co-ordination of patient discharge and to support the development, implementation and evaluation of such processes.
Liaising with Registered Nurses, Doctors, Therapy Services, and Social Services, CHC team, Housing and any other appropriate members of the Multidisciplinary team will be an integral part of your role to provide a proactive and continuous process of discharge planning.
Main duties of the job
1. Act as an effective role model in managing discharges/transfers.
2. Support the team to deliver high quality discharge and patient flow in keeping with the needs and wishes of patients and their families, acting as a patients advocate when necessary;
3. To assist with a multi-disciplinary and multi-professional approach to discharge planning, including the patient and carer in the provision of seamless care;
4. To help to co-ordinate the patient discharge needs and to assist in the development of a discharge plan for individual patients, including evaluating progress to ensure the pathway remains appropriate;
5. Provide information for patients and their family which is relevant, accurate, and timely meets their needs and is understood to enable them to make informed decisions regarding discharge plans;
6. Provide all patients with a Discharge planning leaflet within 24 hours of admission. Under direction update discharge plans on a daily basis. Update patient Estimated Discharge Date on millennium, amending paper and whiteboard documentation and discharge tracker as appropriate;
7. Assist Registered Nurses with discharges and transfers on a day to day basis, liaising with medical staff for completed Discharge Summary, Pharmacy for discharge medications and Bed Managers;
8. Assist Registered Nurses to complete discharge documentation in a timely manner;

* NVQ Level 3 Care Award – Direct Care or equivalent
* Understands the need for strict confidentiality. Experience of working in a Healthcare related environment. Understands and committed to a caring approach to patients, carers and families. Successful achievement of competencies in a Band 2 role. Understands patient flow within a hospital environment.
