Job summary
Assess each residents physical, psychological and social needs and develop up a care plan taking into consideration the lifestyle, gender and background of the people we care for and involve the patient, family and multi-disciplinary team.
Continuously evaluate care needs with other members of the nursing team and ensure that care plans and records are up-to-date and accurate. Continually seek to improve the care delivered.
Give a clear communication of the ageing process and how it affects each persons health and lifestyle. Provide daily visits to assess individual needs and assist service users to maximise their comfort, dignity and independence and create the right environment which supports their physical and mental well-being.
Lead a multi-disciplinary shift team, supervising staff in all respects including orientation of new staff, appraisal, training and development of care staff, organising and co-ordinating their duties, guiding, supporting, monitoring and evaluating staff performance.
Main duties of the job
Youll support adults with lots of different needs such as learning disabilities, autism, ABI and mental health

Youll do activities like cooking, art or listening to music. And get out for movie nights and food outings

Job responsibilities
1. Delegate appropriately but ensure new service users are orientated intotheir surroundings and introduced to other service users and staff, and theappropriate documentation is completed on time.

3. Plan, implement and evaluate nursing care in accordance with the serviceusers needs and within the named nurse system. Ensure the named nursesystem is kept up-to-date.

8. Comply with the NMC Code of Conduct at all times and ensure processesare adhered to for the safe ordering, custody, storing, disposing andadministration of all medication. Accept responsibility for the safety ofmedication in store and on the trolley during dispensing rounds.
9. Manage stocks of all supplies effectively and economically and ensurethe timely order of replenishment stocks.
10. Work co-operatively with activities staff to ensure service usersspiritual and social needs are met.
11. Proactively develop professional and clinical skills in order to enhancethe knowledge and skills needed for safe and effective practice.
12. Contribute to the development of new ideas in nursing and social care inrespect of stimulation for service users to improve their quality of life.
13. Participate in continuous process improvements and make recommendationsfor change with regard to clinical and non-clinical policy.
14. Participate in an on-call rota if assessed to be competent.
15. Assist with the investigation of any complaints.
16. Keep the Home Managers informed of any relevant issues.
17. Maintain detailed an up-to-date knowledge of the Mental Health Act.
18. Phlebotomy ensuring required blood tests are up to date.
19. Ordering and checking in medication.

Experience
Essential
1. Proven leadership skills & abilities
2. Experience of working with families, carers and significant others
3. Ability to carry out under supervision pre-admission assessments
4. Knowledge of mental health and community care legislation and policy
5. Ability to demonstrate initiative. Approachable. Polite. Flexible. Excellent communication skills. Manual dexterity. Good IT skills. Ability to plan and prioritise own and others workload. An understanding of the independent sector and Fieldbay performance targets.
