Senior MH Nurse/Occupational Therapist/Social Worker
Band 6 Community Mental Health Practitioner - Waveney Adult Community Mental Health Team
Waveney Mental Health Team is looking for new Band 6 Community Mental Health Practitioners. You will be a qualified professional Nurse, Occupational Therapist, or Social Worker with experience of working in the community supporting working-age adults with mental health problems following the Care Programme Approach. The successful candidate will demonstrate a sound understanding of their role as a Care Coordinator and join our friendly multidisciplinary team of Social Workers, O.T.s, and Nurses who are experienced community practitioners. You will also be part of the triage and assessment process within the team for referrals from GP surgeries and other services, so you will need to demonstrate robust and sound assessment skills.
Main duties of the job
* To undertake the planning and delivery of direct patient care with minimal supervision, ensuring continuity of practice and working within a multidisciplinary and multi-agency environment.
* To manage a caseload and the provision of mental health care services to people with mental health care needs.
* To be responsible for accurate mental health clustering of all patients on admission, discharge, and at review.
* To ensure all mental health clustering information is passed onto other Trust teams or services on transfer of patients.
* To ensure accuracy and quality of mental health clustering activity within the team.
* Applicants must hold a full UK driving licence.

Person Specification
Qualifications
* Nursing degree or equivalent, Dip OT, Clinical Psychology or equivalent
* Mentorship qualification or ENB 998. Willingness to train to become Sign-off Mentor
* Ongoing professional development working towards degree level
* Higher degree or postgraduate certificate in the area of clinical/therapeutic practice
* Good to great management course, or equivalent
Experience
* Able to demonstrate an appropriate level of knowledge of mental health practice from previous experience
* Demonstrate effective risk assessment and risk management experience
* Experience in the delivery of clinical interventions
* Experience in working in a team delivering person-centred treatment packages
* Experience in delivering group-based interventions
* Experience in supporting service development
