The Medicine for the Elderly clinical pharmacy team is pivotal in preventing hospital admissions and enhancing care throughout the patient pathway. Frail patients, often facing complex health issues and medication regimens, require specialised expertise in medication management. Clinical Pharmacy brings a unique skill set, ensuring that medications are not only optimised for efficacy but also tailored to the specific needs of older frail people.
By conducting and being accountable for comprehensive medication reviews, identifying potential adverse reactions, and collaborating closely with interdisciplinary teams, the newly established MFE clinical pharmacy team plays a crucial role in mitigating medication-related risks. This proactive approach not only prevents unnecessary hospital admissions, reducing the burden on healthcare resources, but also contributes to an overall improvement in the quality of care for older people.
We are seeking an enthusiastic candidate to be a Senior Clinical Pharmacist, working as part of the multidisciplinary team to develop medication optimisation services for patients across Tayside.
You will work to optimise medicines, ensure continuity of care, promote better health outcomes, and foster a patient-centred quality approach.
You will have previous experience of working within a clinical environment. Training will be provided and you will be supported to develop a portfolio and seek accreditation in line with the RPS Core Advanced Pharmacist Curriculum. A commitment to continuing professional development is essential. You must be registered with the General Pharmaceutical Council and possess a prescribing qualification or be prepared to work towards one.
