PURPOSE OF THE ROLE: Provide expert advice to clinicians within a locality (primary care network/s) of 10-15 GP practices on Medicines Optimisation, including policies and procedures to achieve high quality, safe, effective prescribing, and value for money. Facilitate medicines optimisation between patients, prescribers, community pharmacies and secondary care, including providing advice on the Community Pharmacy Contract and commissioning of pharmacy services. Assist the Chief Pharmacist in advising Somerset ICB and wider Somerset System on Medicines Optimisation issues including health service developments, governance, professional issues, formulary development, introduction of new drugs, patient complaints, and pharmacy services.
KEY RESPONSIBILITIES OF THE ROLE:
1. Facilitating medicines optimisation between patients, prescribers, community pharmacies and secondary care.
2. Providing advice on the commissioning of pharmacy services.
3. Working with the Associate Director of Medicines Management to advise Somerset Integrate Care Board (ICB) on Medicines Optimisation issues including Health Service Developments, Governance, Professional issues, formulary development, introduction of new drugs, patient complaints, pharmacy services.
4. Overseeing and managing the implementation of specific changes in prescribing medication, formulary changes and medication reviews within General Practice by employed and commissioned staff.
5. Meeting at least annually with each GP practice within their locality and providing support and influence improvements in the quality, safety and cost effectiveness of medicines optimisation.
6. Providing data to individual practices and federations to assist with peer comparison of medicines optimisation data.
7. Leading on the development and implementation of specific projects as delegated by the Associate Director of Medicines Management.
8. Leading on the cost effectiveness of the commissioned practice support pharmacists and technicians.
9. Providing expert advice to commissioning working groups on areas of medicines involvement in pathways.
10. Providing expert knowledge and input into delegated committees and patient pathway groups e.g. drugs and therapeutics committees.
11. Managing the improvement in the quality, safety and cost effectiveness of prescribing by performance monitoring the implementing specific changes in prescribing medication, formulary changes and medication reviews within the locality group of General Practices.
12. Working closely with the practice manager and prescribing lead of each practice including regular discussions with individual practices within the federations about the annual QOF and Local Enhanced Service targets.
13. Monitoring and working with the practices to ensure the targets are achieved.
14. Supporting the implementation of the ICB medicines optimisation strategy to deliver better clinical outcomes for patients and leading on specific areas as delegated by the Associate Director of Medicines Management.
15. Facilitating the training and improvement of practice staff and support staff, including leading on specific training sessions, contributing to the newsletter and presenting at prescribing leads conferences.
16. Ensuring that CMO/Medicines and Healthcare Products Regulatory Agency (MHRA) and other Drug or safety alerts are reviewed within the practices and federations.
17. Ensuring all support staff communicate effectively and successfully with patients on medicines optimisation including changes to their medication including clinical and cost effectiveness benefits.
18. Leading on the Medicines Optimisation strategy with regard to support provided to social care and care homes.
19. Supporting the ICB in the delivery of the overall strategy including the clinical outcomes framework and NICE quality standards.
20. Supporting the ICB in the delivery of improved clinical outcomes for patients.
21. Supporting the ICB in achieving financial balance and stability.
22. Leading on the support of the investigation of incidents and complaints relating to medicines optimisation.
23. Advising on and witnessing the destruction of Controlled Drugs where appropriate.
24. Analysing high complex prescribing data and using it to influence improvements in individual prescribing practice to improve the overall quality, safety and cost effectiveness of prescribing.
25. Being responsible for the prescribing budget for their specific locality.
26. Leading on the support to prescribers, practices and federations to rectify areas of prescribing and medicines optimisation underperformance.
27. Leading on the work with prescribers, practices and federations to improve systems involved in medicines provision and optimisation.
28. Working with prescribers and practices to identify unmet pharmaceutical needs of patients.
29. Working with designated Secondary Care organisations to agree and implement Medicines Management policies and prescribing practice including joint formulary.
30. Working with other commissioned services and providers to ensure that medicines optimisation occurs across patient pathways and services.
31. Leading on the closer working within localities and between practices and other commissioned services with regards to medicines optimisation.
32. Working with prescribers and practices to identify potential harm being caused by medicines and rectifying to improve outcomes and reduce admissions.
33. Leading on relationships with local acute trust by attending their drugs and therapeutics committee to develop common drugs formulary, agreements for Shared Care, Traffic Light systems and introduction of new drugs in accordance with Prescribing Forum directions.
34. Developing and managing Somerset wide formulary and advising on the setting of prescribing budgets both overall levels and allocation of indicative budgets to practices.
35. Advising on the development of non-medical prescribing including patient group directions.
36. Assisting in the development of Clinical Governance arrangements.
37. Providing professional input to the NICE implementation group, the Clinical Networks and other service development groups.
38. Line managing the pharmacy support technicians (including appraisal) and practice support pharmacists.
39. Having expert knowledge of practice IT systems with regards to medicines and prescribing.
40. Taking clinical lead for medicines optimization in diabetes and respiratory conditions.
