The Role: As a Support Coordinator, you will: Provide respectful and practical care for individuals with physical disabilities and complex health needs across two services. Foster independence and help service users lead meaningful and valued lifestyles. Coordinate and supervise a team to ensure the highest standards of care. Liaise with families, multidisciplinary teams, and other professionals to maximise outcomes for service users.
