We are seeking two enthusiastic part-time fully trained practice nurses to work around 20 hours a week each on either day of the week except Tuesdays, during the opening hours of the surgery. One of the shifts must be a late shift ending at 6pm.
The ideal candidate should be lively and bubbly with a can-do attitude and are used to working in a NHS GP Practice, with care being the forefront of your mind.
We are a small friendly GP practice in the heart of Hayes, Kent, with only 6500 registered patients.
If successful, we would need 2 employment references and an enhanced DBS check prior to commencement of employment.
We do not have an overseas visa support scheme available and will only consider applicants located in the UK who hold the right to work in the UK documentation.

Main duties of the job
Only candidates with primary care/GP experience will be considered.
The practice nurse is responsible for providing nursing services to the practice population, within the boundaries of the role. They will support patients to be healthy, monitor patients with long-term conditions, and deliver health prevention, promotion, and screening activities.
This will include but is not limited to assessing patients; planning and implementing treatment protocols; and reviewing and tracking progress.
* Implement and evaluate treatment plans for patients with long-term conditions.
* Long-term disease management, dependent on level of experience and training, including but not limited to:
* Take and test pathology samples from patients for analysis (blood, sputum, urine, etc).
* Care for patients with uncomplicated wounds, including (but not limited to):
o Removal of sutures
o Wound assessment
o Wound cleaning and dressing
o Management of burns and scalds
o Ulcer care (pressure dressings training not needed)
* Implement and participate in vaccination and immunisation programmes.
* Routine immunisations, (as follows, but not limited to).
* Where qualified, prescribe medication within your areas of competence.
* Perform other clinical duties as required, within the scope of the practice nurse's competencies and training.

We have 2 GP Partners, 2 salary GPs, 1 long-term locum GP, Phlebotomist, admin, and reception team. You will be replacing our long-term registered practice nurse.
We are also part of the Hayes Wick PCN and have access to an array of diverse clinical teams who either work remotely, at a third-party premises, or in-house such as Clinical Pharmacist, Physiotherapists, Mental Health Practitioner, Social Prescribers, and General Practitioner Assistant. They also provide extended services for our surgery, meaning patients have more choice when booking an appointment such as late evening appointments and Saturdays.

* Stock ordering and management: Nursing: dressings, sample bottles, phlebotomy kits, emergency kit, vaccines, sundries for surgery (re. hand towels, toilet tissue, etc.)
* Coding clinical letters, childhood immunisations, heel prick results
* Record accurate consultation notes with applicable QOF and other contract coding, using EMIS X/Ardens Templates
* Working within your scope of practice and knowing when to refer to GP/onwards safely
* Attendance and participation in MDT, Clinical, AHD, and other staff meetings and training events are mandatory
* Cold Chain Management
* Emergency Equipment monitoring

* Liaising with third parties, such as suppliers, our PCN leads, other MDTs
* Self-management and CPD and mandatory training learning and kept up-to-date
* PGD updated and reviewed when needed, and sharing knowledge with others

* Use translating services for patients with accessible needs

Qualifications
* Registered Nurse
* Post Graduate diploma or degree - primary care Non-medical prescriber
Experience
* NHS GP Practice Nurse experience of 2 years +
* Experience of chronic disease management
* Experience of infection prevention and control measures
* Travel consultations experience and certification of course completion
* EMIS X knowledge
* DOCMAN knowledge
* Smear Taker Code with HPV update, cytology knowledge, and safe management follow-up post positive result
* Interpreter of basic blood test results (HbA1c, Lipids, LFT, U&Es and ACR, Cytology)
* DOCMAN coding on EMIS after reviewing clinical letters (Diabetes: DECS, Foot check, Respiratory letters, Cytology)
* Zoladex administration
* EMIS QOF management, including coding, recall, and searches
