Oversee the Pathways Advisor services of The Life Rooms within these areas in various locations across several different workstreams as well as Life Rooms core provision.
Engagement activity to support the mobilisation of the interoperability programme across clinical teams. This programme supports clinical teams to refer to the Life Rooms Services by linking the Elemental Social Prescribing platform to the RiO clinical system. This links clinical teams to a programme of community activity via the Pathways service.
Role involves project implementation and development, line management, community engagement and outreach, and all service operations duties.
This is a permanent position and the post holder will be expected to cover both Walton and Lee Valley Life Rooms footprint. The role involves completing in person engagement with Mersey Cares clinical teams in the Liverpool area., The role of the Community Inclusion Lead is part of the operational management of The Life Rooms Liverpool Pathways Service, in which they are based, working within the Social Model of Health. The role involves working closely with Mersey Cares clinical teams as part of the interoperability programme, and working closely with community partners to support the provision of a service offer focused on reducing health inequalities.
