* To co-produce bereavement recovery plans. This will help reduce crises and alleviate pressure on intensive healthcare services.
* To support the client where necessary by calculating entitlements, negotiating, drafting, and writing letters.
* To provide advice and casework covering a range of topics including (but not exclusively); Welfare Benefits, Housing, Social Care and local services.
* To conduct assessments and provide practical assistance (e.g., pensions, funeral arrangements, respite care), as well as emotional support.
* To direct individuals to other services offered by the partnership and local support mechanisms across Dyfed and Powys, ensuring comprehensive care that aligns with NICE bereavement guidelines.
* To initiate peer support groups during the funding period.
* To provide services that are adaptable and could either be face-to-face in the home/office, and/or telephone to meet the needs of the individual.
* To provide and maintain statistical information and case records in line with data protection regulations.
* To work as part of a team with other advice staff and volunteers.
* To liaise with outside agencies including Social Services and Department for Works and Pensions on behalf of clients.
