An exciting opportunity has arisen to join our team as a Specialist Respiratory Nurse or Physiotherapist.

You will be working with a friendly and quality-driven team, within the multi-disciplinary healthcare setting.
You must be a car driver, with access to a car in order to carry out the role.

This service will be transitioning to HCRG Care Group from the 1st April 2025. Main duties of the job
The post holder will work closely with the existing clinical team who deliver the community pulmonary rehabilitation programme, COPD supported discharge and home oxygen provision for patients within the Swindon area. The post will involve using advanced clinical assessment skills to provide optimal clinical support and assessment for the safe provision of respiratory care for our patients. The post-holder will need to have advanced clinical assessment and treatment skills, and a willingness to support junior staff to develop new skills to achieve a standard of excellence in respiratory management.
Job responsibilities
* Develop care plans and instigate therapy, based on comprehensive patient focused assessments, relevant guidelines and best available evidence in order to improve health outcomes.
* Perform capillary blood gas sampling to assess patients' suitability for Long term Oxygen Therapy (LTOT), interpreting the results and considering the available prescription options available. To ensure cost effective and appropriate provision of oxygen in the patients' home according to national standards.
* Provide specialist assessment and treatment of patients with COPD and ILD.
* Support the delivery of the Community Pulmonary Rehabilitation (PR) service. This will include education to groups of patients/carers on condition-specific and sensitive information as well leading exercise programmes.
* Promote patients to self-manage their condition and encourage behavioural change strategies by identifying where non-pharmacological interventions are appropriate, e.g. smoking cessation, pulmonary rehabilitation, home exercise regimes and improved concordance with medication.
Person Specification
Education, Qualifications and Training
* Registered Nurse or Physiotherapist
* Qualified to degree level
* Expert knowledge in the treatment and management of long-term conditions
* Knowledge of the use of pharmacology in the management of long-term disease.
* COPD diploma or equivalent knowledge, skills and experience
* Non-medical prescriber or willingness to work toward this qualification
Experience
* Experience of working with a multi-disciplinary team
* Setting and monitoring standards of care
* Experience within a wide range of patient/client care groups
* Experience working with patients with long term respiratory conditions
* Profiling community needs and targeting identified health needs of population
* Evidence of innovative and flexible approach to care and the organisation of care
