Job summary This post is not available for sponsorship To support patients, their families and carers to maximise their participation in the attainment of their rehabilitation goals by following treatment programmes developed and provided by the Allied Health Professionals (AHP) involved To work as a supervised member of the multidisciplinary team, maintaining good communication with the AHPs, and other members of the multi-disciplinary team e.g. doctors, nurses and social workers to try to achieve the Predicted Date of Discharge (PDD) Main duties of the job Gain valid patient consent and understanding of rehabilitation and educationprogrammes prior to any intervention involving families and carers where necessary and respecting patients' right to decline treatment. Have the capacity to work within a legal framework with patients who lack capacity to consent to treatment.
