Primary Care Advanced Nurse Practitioner (ANP)
In recent years, the role of General Practice and Community Care has expanded at an exponential rate. Traditionally, GPs with a small team were able to meet the vast majority of their patients' needs. Over time, societal change, population increases, and role expansion have changed the face of General Practice forever. We are seeking to add an additional experienced Nurse Practitioner to join our modern multidisciplinary team. We would like an ANP to work with our Same Day Assessment Team. This job will involve triage, clinical assessment, prescribing, and provision of same-day care to our practice population.
Main duties of the job
1. Ability to independently deliver care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment, and evaluation of care.
2. Deliver safe clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the General Practice.
3. To take on, solve, and reduce GP workload directly within the scope of their clinical practice.

Job responsibilities
1. Assess patients presenting with acute illness or for review of chronic disease conditions.
2. Skills and ability to consult and triage: Face to Face, by Telephone, at Home Visit, and by Video Consultation.
3. Utilize practice IT (SystmOne TPP) for consultation and all other aspects of clinical care, including recording clear, competent consultation notes and care plans.
4. Diagnose, plan, implement, and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis.
5. Clinically examine and assess patient needs from a physiological and psychological perspective and plan clinical care accordingly.
6. Assess diagnosis, plan, implement, and evaluate interventions/treatments for patients with complex needs.
7. Proactively identify, diagnose, and manage treatment plans for patients at risk of developing a long-term condition (as appropriate).
8. Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan.
9. Fully participate in the clinical administration within the practice, signing prescriptions, reviewing medication, EPS.
10. Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols and formularies.
11. Work with patients and their carers in order to support compliance with and adherence to prescribed treatments.
12. Provide information and advice on prescribed or over-the-counter medication on medication regimens, side effects, and interactions.
Person Specification
Experience
* Some working experience in Advanced Nurse Practitioner Role.
* Experience and ability to work independently.
* Working knowledge of General Practice.
* Broad nursing experience and qualification.
* Respiratory Special Interest and associated Experience.
Qualifications
* Nursing Qualification (degree level or equivalent).
* Advanced Nurse Practitioner Qualification (MSc or equivalent).
* Ability to consult with patients via:
o Face to Face Assessments.
o Telephone Triage and Consultation.
o Video Triage and Consultation.
o Home Visit Consultation and Assessments.
* Registration with NMC.
* Independent prescriber Qualification or steps towards this.
* Further Post Graduate Qualifications or Diplomas.
Knowledge and Skills
* Ability to work independently within a Primary Care team.
* A knowledge and understanding of General Practice that empowers the Advanced Nurse Practitioner to understand how to take on and reduce workload from other clinicians.
* Ability to organize and prioritize own work.
* Good communication and networking skills.
* Ability to problem solve variable situations and variables.
* Understanding of the wider primary care health and social aspects.
* Knowledge of local services and referral pathways.
