Job overview
This is a new and exciting post within the Department of Medicine for the Elderly (DME)/Outliers. The successful candidate will work with existing substantive Consultants to provide a comprehensive service for elderly patients and medical outliers across both acute and community settings.
The post holder will support the delivery of an Acute Frailty specialist service as part of the Needs Based Care Model implemented across the Trust, ensuring that emergency attendees are reviewed by the appropriate specialist.
This role offers an opportunity for an enthusiastic and energetic geriatrician to play a key role across multi-disciplinary teams, focusing on developing responsiveness and capacity in services for older people and outlying medical patients with complex needs in the Luton & Dunstable area.
The on-call physician in medicine for the elderly will act as the site medical co-coordinator during major disasters. The post holder will contribute to enabling older people to live independently, empowering them to manage their conditions and reduce the need for acute hospital interventions.
Main duties of the job
1. Maintain the highest clinical standards in managing all patients presenting to the hospital, optimizing safe and timely discharge, and supporting patient flow.
2. Share responsibilities with colleagues for managing complex patients referred to the Trust, providing senior medical input for frail elderly patients in the Emergency Assessment Unit and Emergency Department.
3. Collaborate with partner organizations to support integrated care development, provide specialist guidance to community MDT teams, and manage patients via rapid access clinics.
4. Participate in departmental and Trust Clinical Governance and audit activities.
5. Engage in continuing professional development (CPD).

Person specification
Training
* Training and experience equivalent to that required for UK CCT (or within 6 months of issuance). Up-to-date ALS.
Registration
* Entry on GMC Specialist Register in Geriatric Medicine / Geriatrics and General Medicine or eligibility to enter within 6 months of the interview, holding a Licence to Practice.
Qualifications
* MB ChB or equivalent
* MRCP
* MD or equivalent higher degree or evidence of relevant research
Clinical Experience
* Evidence of thorough training and experience in Geriatric and General Medicine.
* Able to provide expert clinical opinion on all aspects of Geriatric and General Medicine.
* Capable of independent service delivery without supervision.
* Expertise and leadership in a sub-specialty area of Geriatric Medicine.
Teaching
* Knowledge and ability to teach all grades of trainees and medical students.
* Completion of a teaching the teachers course or similar.
Clinical Governance
* Broad understanding of clinical governance principles and elements.
Communication and language skills
* Excellent verbal and written English skills.
* Effective communication with clinical colleagues and support staff.
* Residence within 10 miles or up to thirty minutes of the hospital for on-call emergencies.
