Wigtownshire Community Health and Social Care (CHSC) team is looking for a motivated, proactive and enthusiastic pharmacist to work within our Pharmacy Hubs and GP practices. Our friendly team of pharmacists, pharmacy technicians and pharmacy support workers make up the GP practice clinical pharmacy service, taking a leading role in medicines management and medication review as part of the primary care multidisciplinary team. It is a rewarding role, helping patients in Dumfries and Galloway to safely manage their medicines.
You will provide a clinical pharmacy service directly to patients. Day-to-day responsibilities might include: clinical assessment and review of patients, performing complex polypharmacy medication reviews with patients, dealing with medicines related queries, assessing patients ability to manage their medicines (including in the patients home), medicine reconciliation and prescribing of medication. Joint working with GP practice staff will be routine, as well as with the multidisciplinary teams within the local hospitals, Community Pharmacies and Home Teams.

This is a fixed term post, for 9 months, and will be for 22 hours per week based in Wigtownshire GP practices. We are keen to offer opportunities that suit your circumstances and are happy to consider hybrid working models. Additional hours may be available if desired.

NHS Scotland is reducing their full-time working week from 37.5 to 37 hours per week for NHS Agenda for Change workers in Scotland from 1 April 2024 but with no change in pay. This reduction will also be applied pro rata for part time staff. This advert and any subsequent offer/contract of employment therefore reflects the new working hours. However, as not all service areas will be able to adopt the 37 hour working week immediately from 1 April 2024, you may be required to work up to an additional 30 minutes per week for a temporary period, for which you would be paid, until the service you are working in changes rosters or working patterns to accommodate the new reduced working week.
