Job summary Ambrose Avenue Group Practice, based in Colchester, has a fantastic opportunity for a healthcare assistant to join their busy team on a full-time basis. Working alongside our existing HCAs and practice nurses, the primary role of the HCA will involve caring for patients with long-term conditions, managing wounds, and ensuring infection prevention and control. You will work alongside our practice nurses, HCAs, GPs, ANPs, pharmacist, and other healthcare professional within our primary care network. The role will offer you the opportunity to build on your experience, skills and competence, requiring you to work effectively within a multidisciplinary healthcare team. Main duties of the job The healthcare assistantwill be able toapply their clinical skills and knowledge to carry out high quality practice within designated clinical areas such as: Provide general and specific screening to practice patients. Assist with managing and developing care programmes for individual patients within specific long term condition group. Participate in quality audit initiatives, enhanced services and vaccinations, to include in annual home visits for housebound patients. Plan and implement health campaigns. Carry out immunisations, chronic disease management, e.g. diabetes. Give advice on healthy living. Conduct NHS health checks and managing INRs.
